We are a faith-based, non-profit home health and hospice agency. Triniti Home Health & Hospice is jointly owned by Guardian Angels Senior Services and Cassia, an Augustana/Elim affiliation. From the comforting care we provide in hospice, to the restorative focus we have in home health care, our high-powered teams set us apart from other agencies. Triniti Home Health & Hospice employs a community of health care professionals consisting of nurses, musical therapists, massage therapists, volunteers, and more. Whether you or your loved one is looking for care, our compassionate team is equipped to handle a variety of needs.
We serve people in Anoka, Carver, Dakota, Hennepin, Isanti, Mille Lacs, Ramsey, Scott, Sherburne, Washington, and Wright counties in Minnesota.
